Altius Health Plans wants to provide quality care for you and your family. This directory lists the medical providers affiliated with Altius. Our providers include some of the most respected health professionals in the Intermountain West. Our provider search function is updated weekly. While it is our goal to provide accurate information, the composition of our provider network is subject to change without notice. The participation of a particular provider is never guaranteed, even if their physician refers a member to the provider. Altius is in the process of negotiating with additional physicians and hospitals, and changes will be made as appropriate. It is important to know when you enroll in this plan, services are provided through the plan's delivery system, but continued participation of any one doctor, hospital, or other provider cannot be guaranteed.

You may be entitled to coverage for health care services from the following non-participating providers if you live or reside within 30 paved road miles of the listed providers, or if you live or reside in closer proximity to the listed providers than to Altius participating providers:
- Green Valley/River Clinic, Green River, Emery/Grand Counties, Utah
- Halchita Clinic, San Juan County, Utah
- Monument Valley Health Center, Monument Valley, Utah
This list may change periodically. Please check our website or call for verification. Please be advised that if you choose a non-participating provider you will be responsible for any charges not covered by your Altius benefit plan.
